This Application Form accompanies the Product Disclosure Statement for the Universus Master Income Fund (the Fund) dated 15 May 2026, which is issued by One Managed Investment Funds Limited ACN 117 400 987 as responsible entity of the Fund. The Product Disclosure Statement contains details of an offer to acquire units in the Fund. An application for units in the Fund, if successful, involves making a financial investment. Accordingly, it is important that you read and understand the terms and conditions of the offer for the financial product, as set out in the Product Disclosure Statement and the accompanying Application Form, before applying for Units. Unless otherwise defined, capitalised terms used in this Application Form have the same meaning given to them in the Product Disclosure Statement.
